Our plans offer comprehensive coverage to keep you feeling your best, with options designed to fit your needs and automatic contribution withdrawal from your paycheck.
Do you prefer predictable costs and a primary care provider to guide your care? An HMO (Health Maintenance Organization) plan offers coordinated care within a network of trusted providers.
Do you prefer more flexibility in seeing specialists without a referral? A PPO (Preferred Provider Organization) plan allows you to choose doctors in and out of the network.
No matter your preference, our medical plans are built to support your well-being every step of the way.
You are eligible for benefits if you work 30+ hours weekly.
ELIGIBILITY
For new hires, benefits are effective on the first of the month following the hire date. You may also enroll your eligible family members under certain plans you choose for yourself. Eligible family members include:
Your legally married spouse
Your domestic partner (DP) and/or their children, where applicable by state law
Your biological children or children for whom you have legal custody (age restrictions may apply)
Disabled children age 26 or older who meet certain criteria may continue on your health coverage

Available Medical Plans
Everbright offers several medical plans for employees to choose from to meet their medical needs. You are able to choose and make changes to your plans during our annual enrollment period or when qualifying life events occur (i.e., marriage/divorce, birth or adoption of a child, coverage loss of alternate insurance).
Making Changes
You must contact Jenifer Redding at jenifer.redding@hubinternational.com within 31 days of a qualifying life event to change your plan.
You may need to provide documentation, such as a marriage license, birth/death certificate, or divorce decree. If changes aren’t submitted on time, you must wait until the next open enrollment period to change your elections.
